Fat deposits on the legs – unwanted companions

With increasing age, fat pads often appear on the knees and thighs. In women, excess fat cells are particularly stubborn on the thighs, which is why the so-called saddlebags – fat deposits on the buttocks and outer thighs – form there, defying even the most intense sports program.

The reasons for this lie in genetic predispositions, but above all in evolutionary and hormonal factors: the body stores fat reserves in these areas, which originally served to safely carry mother and child through a pregnancy even in times of need. In other cases, not only the thighs and buttocks, but the entire leg can be affected by the unwanted fat deposits.

Leg liposuction is performed in one or more of these problem areas. However, treating different zones is sometimes also necessary for aesthetic reasons – for example, if a combined liposuction of the outer and inner thighs is performed, the leg can quickly appear columnar due to the now significantly narrower thigh.

Knee liposuction in this case restores normal and aesthetically pleasing proportions. The general goal of leg liposuction is a sustainable optimization of the leg contour in relation to the other body proportions.

What results can be achieved with leg liposuction?

Through liposuction, your legs regain their original slender, firm shape - or even gain it for the first time. The first effects of liposuction are already visible in the first few days after the procedure. As swelling and bruising subside, the visual result continuously improves in the coming weeks. Your legs usually reach their final contour within eight to ten weeks, but in individual cases, a period of up to six months may be necessary for this.

Procedure of leg liposuction in Berlin

Leg liposuction usually takes place on an outpatient basis; a – usually one-day – hospital stay is possible at your request, due to individual risk factors or due to the extent of the procedure. To prepare for your procedure, we conduct an individual consultation before the treatment – should there still be questions after this, there is always the possibility of a further consultation.

Leg liposuction can be performed using various methods – however, the principle of the procedure is identical for each method. The subcutaneous fat tissue is selectively dissolved and then suctioned off with thin cannulas. The removal of excess fat deposits can be precisely controlled, making it possible to achieve aesthetically optimal results with leg liposuction.

Before the actual liposuction, the fat deposits to be treated are marked with a body pen in the form of contour lines and the incision sites for the cannulas are defined. If the skin in the leg region to be treated was already lax before the procedure or if there is too much excess skin due to the removal of body fat, liposuction can also be combined with a skin tightening.

If it is possible due to the individual circumstances of the patient, the J-Plasma-Renuvion procedure is used for this purpose. This is an innovative technology that allows non-surgical skin tightening by generating a cold plasma jet – the so-called Cool-Plasma – through electrical energy and helium gas.

The energy released in this process acts directly on the connective tissue and causes lasting tightening, without damaging the surrounding tissue. For this purpose, the probe of the plasma device is guided under the skin through the skin incisions made for liposuction. The plasma jet is generated in the handpiece of the device and can be regulated so that every part of the treatment area is reached.

In addition, it may be possible to use the removed fat cells for another cosmetic surgical procedure, such as breast augmentation by means of lipofilling (autologous fat transplantation). In this case, the two operations are coordinated in terms of timing – but more details will be discussed in a consultation.

Pre- and post-operative care for leg liposuction in Berlin

To avoid complications, you should not take blood-thinning medications – anticoagulants or painkillers such as aspirin – one week before the procedure. If you also refrain from alcohol and nicotine during this time or reduce their consumption to a minimum, you can significantly reduce the risk of wound healing disorders.

Immediately after leg liposuction, you will receive compression garments in our practice, which you should wear continuously for the next four weeks – including at night. This prevents the cavities created by the liposuction from filling with blood, tissue fluid, or dissolved fat cells.

This could lead to the formation of scars or seromas, which not only jeopardize the aesthetic result of the liposuction but, in the worst case, may even require surgical intervention. During this time, you should also absolutely refrain from sports and other physical exertion. Showers are possible for the first time on the second or third day after liposuction; you should avoid full baths and sauna visits for four to six weeks.

In the first few days after liposuction, slight pain, bruising, and swelling in the treated leg regions are normal. Pain can be reduced to a minimum with medication, and bruising and swelling can be alleviated by regular cooling. Swelling may only completely subside over several weeks or months.

Follow-up examinations after leg liposuction are carried out in our practice throughout the healing process by individual arrangement. In case of severe pain, fever, inflammation, or hardening in the treated body areas, please contact us immediately.

Risks of leg liposuction

Liposuction is generally considered a low-risk operation. If the procedure is performed by an experienced specialist in plastic and aesthetic surgery, complications and undesirable side effects are usually hardly to be expected. However, as with any other surgical procedure, leg liposuction can lead to bleeding, bruising, infections, wound healing disorders, or medication intolerances.

In the first few weeks after the procedure, changes in touch sensitivity or numbness may occur in the treated body areas, but these are usually only short-lived. Dangerous complications such as embolisms are extremely rare with liposuction. Worldwide studies show that complications with liposuction primarily arise when more than three liters of fatty tissue have been removed or other operations were performed on the day of the procedure that were not related to the liposuction.

As a specialist practice for plastic and aesthetic surgery, we pay very close attention to adhering to these regulations and limits. If it is necessary to remove larger amounts of fatty tissue, a second liposuction will be performed as soon as the healing process of the first procedure is complete.

Where are incisions necessary in the surgical area?

The cannulas for liposuction are guided to the fat deposits through fine skin incisions. Only a few incisions are necessary per surgical area. Suturing these incisions is not always required – if suturing is necessary, the stitches are removed about one week after liposuction. Fine scars may form at the incision sites, but these quickly fade and are barely visible after a few weeks or months.

Depending on the surgical procedure, in addition to the incisions for liposuction, injections of a sterile saline solution may also be necessary to detach the fat cells from the connective tissue of the legs.

Different procedures for leg liposuction

Different procedures are available to the plastic-aesthetic surgeon for leg liposuction:

- Power Assisted Liposuction (vibration-based)
- Water Assisted Liposuction (water-based, WAL)
- Ultrasound and laser-based surgical procedures

Power Assisted Liposuction is the most commonly used procedure for liposuction. It is vibration-assisted and is primarily used today in conjunction with tumescent anesthesia. In this liposuction method, the suction cannula is connected to an electric motor that gently vibrates it at approx. 5,000 revolutions per second.

Before the start of the treatment, a saline solution containing a local anesthetic is injected under the skin of the treatment region. The solution supports the detachment of fat cells from the connective tissue of the treated body region. The liposuction then removes the compact fatty tissue and reduces the subcutaneous fatty tissue.

In Water Assisted Liposuction (WAL), the fat deposits are gently separated from the surrounding tissues by a pulsating, fan-shaped high-pressure water jet and simultaneously suctioned off. In liposuction using ultrasound, the fatty tissue is liquefied and then suctioned off, while laser lipolysis is a procedure that is primarily used for smaller fat deposits or as a supplement to classic liposuction. The fatty tissue is not suctioned off but dissolved by high-energy laser beams.

Frequently Asked Questions

For medical reasons, general anesthesia is not necessary for leg liposuction. It can, of course, be performed at individual request. Local anesthesia reliably eliminates pain during the procedure. The administration of a sedative supports your psychological relaxation or puts you into a twilight sleep.

Inpatient leg liposuction may be useful in certain individual risk situations. These include pre-existing conditions or a larger procedure that exhausts the medically permissible maximum limits of liposuction. Normally, outpatient treatment is not associated with greater risks. Afterwards, you will remain in my practice for some time for observation.

The result of the procedure is permanent. In the operated areas, the fat cells are completely removed, so that no new fat deposits can form there, as long as you maintain your body weight stably.

Although the removed fat deposits no longer count as overweight, your weight status does not change with liposuction. Liposuction is an exclusively aesthetically oriented procedure. If you gain or lose a lot of weight afterwards, this can result in negative effects on your body proportions. It is optimal if you have reached your desired weight at the time of liposuction and have maintained it for some time.

Contraindications for leg liposuction include certain pre-existing conditions, the mandatory intake of certain medications, and very severe overweight. What solutions there are for this is the subject of individual consultation in my practice.

For lipedema, liposuction on the legs should ideally be performed before pregnancy, as fat deposits can increase during this time.
