Reasons for Breast Implant Removal

There are various reasons why you may wish to have your breast implants removed:

Physical or emotional discomfort: Some patients find that they cannot become comfortable with the presence of an implant and experience persistent physical unease. If you are unhappy with the change in the appearance of your breasts, this may also affect your confidence. Breast surgery should help you feel more at ease with your body, not less. In such cases, implant removal may be appropriate.

Changing aesthetic preferences: Your preferences may change over time, and you may wish to return to a more natural breast shape. Having chosen implants in the past does not mean that you must retain them indefinitely. Aesthetic priorities are personal and can evolve throughout life.

Changes in breast shape: Pregnancy, natural ageing and substantial changes in weight can all alter the shape of the breasts. The skin may become lax, and the additional volume of the implants may no longer suit your proportions or preferences. Damage to the implant: Some older-generation implants are more prone to rupture and may cause medical problems. Earlier implants containing less cohesive silicone gel may leak if their shell is damaged. How Does a Capsule Form Around a Breast Implant?

The body normally forms a thin layer of connective tissue around any breast implant as part of its natural response to a foreign material. This capsule is generally soft and does not cause health problems. In capsular contracture, however, the tissue becomes unusually thick and tight, which may cause pain, firmness and changes in breast shape.

The breast may also become less mobile, potentially affecting everyday comfort. Capsular contracture is thought to be associated with several factors, including low-grade bacterial contamination and biofilm formation on the implant surface. Bacteria may be introduced when the implant is inserted or reach the area through the bloodstream. The resulting immune response can contribute to the continued formation and contraction of capsule tissue.

The Treatment Process

If you decide to have your breast implants removed at my Berlin practice, the process begins with a detailed consultation. Before Surgery

Before surgery, I recommend an ultrasound examination and a basic blood test.

During Surgery

On the day of your procedure, you should arrive at the Berlin practice well rested and in good health. The operation is performed under general anaesthesia and usually takes between one and two hours, depending on its extent. To remove the breast implants, I generally make an incision along the inframammary fold beneath the breast. Wherever possible, I use the existing scar from the original implant surgery to avoid creating an additional scar. Alternatively, access may be gained through an incision along the lower edge of the areola.

If capsular contracture is present, the affected capsule can also be removed as part of the procedure. Once the implants have been removed, I reshape and redistribute the glandular and breast tissue where appropriate. If the skin has become lax due to ageing or substantial weight loss, leaving excess skin after implant removal, this can be addressed with a breast lift during the same operation. Removal of extensive capsule tissue may result in breasts that appear smaller and less firm.

If you would like to restore some volume using your own fat, fat transfer can be performed directly after implant removal. The incision is then closed with dissolvable sutures.

Aftercare

After surgery, you will usually remain at the practice overnight for observation. You will be given a supportive bra to wear for the following weeks, removing it only as instructed, including for showering. During the first few days, you should rest thoroughly and allow your body time to recover. Cooling packs may help relieve initial swelling and discomfort. Many patients feel ready to return to social activities after two to three days.

You should avoid physically demanding work for approximately 14 days, whilst desk-based or other non-physical work may often be resumed after around five days. Exercise should be avoided for two weeks, and direct chest training should ideally not be resumed for approximately four months.

Potential Risks

Like any operation, breast implant removal carries potential risks and complications.

1.Infection: Although uncommon, infection can occur after surgery. Treatment with antibiotics or, in some cases, further surgery may be necessary.

2.Bleeding and haematoma: Bleeding may occur during or after the operation, potentially causing swelling, pain and a collection of blood known as a haematoma. Occasionally, further surgery is required to control the bleeding or evacuate the haematoma.

3.Pain and discomfort: Some pain, swelling and discomfort are expected after surgery but should gradually subside as healing progresses.

4.Scarring: Breast implant removal leaves scars. Their size and appearance vary between individuals and depend on factors such as personal healing characteristics and the surgical technique used. Wherever possible, I remove the implants through the existing scar from the original implant surgery to avoid creating an additional scar.

5.Changes in breast shape and contour: After implant removal, the breasts may not return fully to their original shape or size. Irregularities, asymmetry or an appearance that does not meet your expectations may occur. In some cases, an additional procedure such as a breast lift may be recommended to improve the result.

6.Changes in breast sensation: In some cases, breast implant removal can cause temporary or permanent changes in breast or nipple sensation.

7.Anaesthetic risks: As with any surgical procedure requiring anaesthesia, there are risks associated with the anaesthetic itself, including allergic reactions and breathing difficulties.
