What Is Botox?

How Does Botulinum Toxin Type A Work?

After injection, botulinum toxin type A reaches the intended treatment area and binds to the nerve endings, or synapses, of the muscles. It reduces the transmission of signals from the nerve cells to the muscles by limiting the release of the neurotransmitter acetylcholine. Depending on the dose, muscle contraction is reduced or temporarily blocked. This relaxes the treated muscles and smooths the overlying dynamic lines for several months. Because of its muscle-relaxing properties, botulinum toxin type A is used not only for wrinkle treatment in aesthetic medicine but also for a range of therapeutic purposes. These include the treatment of migraine, excessive sweating (hyperhidrosis) in different areas of the body and, in some clinical settings, depression. In dentistry, it may also be used to treat bruxism, or teeth grinding.

What Results Can Botox Wrinkle Treatment Achieve?

Botox wrinkle treatment can temporarily smooth forehead lines, frown lines and other dynamic wrinkles, helping the face look fresher and more rested. Initial results usually become visible two to four days after the injections.

Botulinum toxin type A generally reaches its maximum effect around two weeks after treatment. The body then gradually breaks down the active ingredient, although the rate varies considerably from person to person. On average, results remain visible for four to six months, after which repeat treatment may be considered. By reducing the repeated muscle activity that contributes to dynamic wrinkles, Botox may also help slow the development of more deeply established lines. For suitable patients, I may therefore recommend treatment at an earlier stage, before pronounced lines have formed.

Which Wrinkles Can Be Softened with Botox?

Botox wrinkle treatment is particularly suitable for expression lines—wrinkles caused by repeated muscle movement. These develop because the same muscles and areas of skin are repeatedly engaged by facial expression. In aesthetic medicine, they are also known as dynamic wrinkles. Typical examples include forehead lines, laughter lines around the eyes and frown lines between the eyebrows. Their depth and prominence vary considerably between individuals. Botox is used not only for forehead lines, frown lines and other wrinkles around the eyes, but in selected cases also for lines around the mouth and nose, as well as on the neck and décolletage. Botulinum toxin type A tends to be most effective before lines become deeply established. Pronounced dynamic wrinkles may benefit from a combined treatment using Botox and dermal fillers such as hyaluronic acid.

Botox wrinkle treatment can soften the following types of lines:

Horizontal forehead lines
Forehead lines are caused by raising the eyebrows. They run horizontally across the forehead and are among the most common dynamic expression lines. Botox can be used both to treat forehead lines and to help prevent them from becoming more pronounced. The dose is determined by the number, depth and pattern of the lines, the strength of the muscles and the intensity of your facial movement. Treatment usually involves six to eight individual injections. Men generally require a slightly higher dose than women because of their greater muscle mass.

Frown lines
Frown lines, also known as glabellar lines, form between the eyebrows. They usually appear as two or three parallel vertical lines and are caused by strong muscle activity in this area. Many people draw their eyebrows together when concentrating or in bright light; over time, the resulting lines may remain visible even when the face is at rest. Frown lines are among the most frequently treated areas. Treatment generally involves three to five injections. The temporary reduction in muscle activity may also help prevent the lines from becoming more deeply established.

Laughter lines (crow’s feet)
Laughter lines at the outer corners of the eyes—commonly known as crow’s feet—can look warm and expressive when you smile, but may also make the face appear older. Botox can soften these lines effectively. If they are deeply established, combining Botox with a dermal filler may produce a better result.

Nasal and lip lines (bunny lines and barcode lines)
Nasal and lip lines are also caused, at least in part, by facial muscle activity. Nasal lines form across the bridge and sides of the nose through repeated scrunching, whilst lip lines develop through movement of the muscles around the upper or lower lip. Earlier treatment may help prevent these lines from becoming deeply established. Pronounced lines around the nose or lips are also influenced by age-related changes in the skin, so a combination of treatments may be appropriate. The number of injections depends on the number and extent of the lines.
