What is a wrinkle injection?

Wrinkle injection is a minimally invasive procedure for which various methods and techniques are suitable. The best known is wrinkle injection with hyaluronic acid, which is widely used in plastic-aesthetic medicine for wrinkle treatment and tissue lifting. In addition to hyaluronic acid, various other fillers – such as poly-L-lactic acid or autologous fat – can also be used for wrinkle injection to achieve the desired anti-aging effect.

In contrast, an anti-aging treatment with Botox aims to dampen muscle movements in the face to relax mimic wrinkles.

Such a wrinkle injection is suitable for women and men who want to optically stop aging but desire a gentle anti-aging treatment without surgery. The results of a minimally invasive wrinkle treatment can often compete with the results of a classic facelift. In the following, you will read about the possibilities for this anti-aging treatment.

What are fillers and what variants are there?

Fillers are various medical materials that are injected into the skin as a liquid or gel to create a smoothing and firming effect. Wrinkle injection lifts the skin from below, thus compensating for volume loss.

Fillers such as hyaluronic acid or poly-L-lactic acid are substances that the human body also produces and are therefore particularly well tolerated. However, over time, they are biologically broken down by the organism, so the effect of the anti-aging treatment will diminish sooner or later, and a repeat treatment will be necessary.

At least theoretically, a minimally invasive wrinkle treatment is also possible with non-biodegradable substances such as silicones, which are capable of permanently smoothing wrinkles. However, such substances no longer play a role in anti-aging treatment today, as they can cause rejection reactions and tissue changes. With natural substances, such risks and undesirable side effects are virtually excluded.

Natural fillers for wrinkle injection

The most important natural fillers for wrinkle injection are hyaluronic acid, poly-L-lactic acid, and autologous fat. Which filler is used depends on individual factors – for example, your skin type, your age, and the severity of the wrinkles to be treated. Of course, your personal wishes also play a role in the selection. Depending on the findings, combinations of different fillers can also be used for anti-aging therapy.

A wrinkle injection with hyaluronic acid eliminates wrinkles gently and sustainably. In small doses, hyaluronic acid is particularly well suited for treating fine lines. With higher dosages, more pronounced wrinkles can also be sustainably and visibly softened. An improved appearance through modeling of the facial contour is also possible with this active ingredient.

What is hyaluronic acid?

Hyaluronic acid or hyaluron is a natural anti-aging active ingredient that is also produced by the organism itself. In particularly high concentrations, it is found in the skin, muscles and joints, and in the eyes. Its most important task within the body is to regulate the moisture balance of body tissues on a cellular basis. It also counteracts inflammation.

Hyaluronic acid is able to bind large amounts of moisture. This gives the skin fullness, elasticity, and firmness. However, with increasing age, the body produces ever smaller amounts of this active ingredient, which leads to wrinkles and volume loss.

In plastic and aesthetic medicine, hyaluronic acid is used in many ways. Its main area of application, however, is in the injection of static wrinkles to achieve a sustainable anti-aging effect. Deep nasolabial folds can be treated just as effectively with hyaluronic acid as fine lines around the eyes or lip wrinkles. Further areas of application include the correction of dark circles under the eyes, nose, chin, and jawline corrections, as well as the modeling of the lips and facial contours.

How does hyaluronic acid work against wrinkles?

A wrinkle injection with hyaluronic acid works in two different dimensions: The wrinkles themselves are filled in by the volume of the active ingredient. Skin depressions and sagging skin areas are padded, making them appear smooth and significantly firmer. This anti-aging effect of a hyaluronic acid treatment is visible immediately after the procedure. At the same time, hyaluronic acid develops a long-term effect by improving the binding of moisture in the skin, which also results in a sustainable lifting effect and an improvement in skin structure.

In addition, hyaluronic acid is able to promote skin regeneration. It stimulates skin renewal and the formation of collagen, which is essential for healthy, firm connective tissue. Hyaluronic acid has been used for medical therapies and anti-aging treatments since the mid-1990s. Various preparations are available for wrinkle treatment with hyaluronic acid, the selection of which depends on the desired treatment result.

Criteria for the application of the various hyaluronic acid preparations for wrinkle treatment include, for example, whether the focus should be on wrinkle smoothing or on modeling and volume building.

Also important for the choice of preparation is the cross-linking degree of the active ingredient: hyaluronic acid is available in non-cross-linked, lightly cross-linked, or highly cross-linked form. Highly cross-linked hyaluronic acid preparations are more elastic than other variants, are broken down more slowly by the body, and are therefore primarily used for injections into the deeper layers of the skin. Lightly cross-linked hyaluronic acid, on the other hand, is primarily suitable for anti-aging therapies where fine lines on the skin surface are to be smoothed.

Wrinkle injection with poly-L-lactic acid

Just like hyaluronic acid, poly-L-lactic acid (PLLA, Sculptra) is also a natural and biodegradable filler. It is produced on the basis of lactic acid, which occurs in the human organism, among other places, in the blood and muscles. A wrinkle injection with poly-L-lactic acid restores elasticity and volume to the skin by building up the body's own collagen. This substance is therefore able to smooth out even deep wrinkles, providing a non-surgical tightening.

Another procedure for wrinkle injection that I offer in Berlin is the so-called Liquid Lifting, which combines various injection techniques and fillers. Wrinkle smoothing and facial contouring are achieved through precisely balanced injection points and different injection depths. With a Liquid Lifting, results can be achieved without a scalpel that were originally only possible through surgery. Liquid Lifting can rejuvenate individual facial regions or the entire face.

Eight-point Liquid Lifting

A special variant of this anti-aging measure is the eight-point Liquid Lifting (Profhilo) originating from Brazil, in which the entire face is treated and injections are made into the skin at a total of eight key points that are crucial for a youthful appearance. The fillers are injected into the following skin areas:

  • Cheekbones and the mid-cheek region
  • Zygomatic arch and cheek transition
  • Chin and Jawline
  • Mouth and nose region for smoothing nasolabial folds and wrinkles between the corner of the mouth and the chin

Fillers for a Liquid Lifting

Fillers for a Liquid Lifting include hyaluronic acid, Radiesse, and partly also poly-L-lactic acid (Sculptra). Radiesse is a calcium-based biodegradable filler used for injecting deep wrinkles and has a longer duration of action compared to hyaluronic acid. Hyaluronic acid, on the other hand, is primarily used for injecting superficial wrinkles in a Liquid Lifting.

An innovative filler used in Liquid Liftings is Ellansé. This is a synthetically produced substance based on polycaprolactone (PCL), which is particularly long-lasting and, like Radiesse, is excellently suited for treating deep wrinkles. However, the tolerability of this – also biodegradable – polyester filler has not yet been tested in long-term studies.

As an excellent alternative to wrinkle treatment with hyaluronic acid, I also offer you wrinkle injection with autologous fat in Berlin. The treatment proceeds in three steps: First, autologous fat is removed from the thighs or other suitable body regions as part of a liposuction and prepared accordingly. Subsequently, the actual wrinkle injection is performed by injecting the autologous fat.

An injection of autologous fat is particularly suitable for volume building in larger areas.

In a wrinkle injection with Botox, no filler is used. The smoothing effect results from the fact that botulinum toxin A (short: Botox), depending on the dosage, reduces or completely inhibits the activity of the treated muscles. In its natural form, Botox is a strong neurotoxin. However, Botox preparations for medical applications are manufactured under strictly controlled conditions.

For wrinkle smoothing, Botox is injected into the muscles whose contractions are to be reduced. It attaches to the nerve endings of the muscles. This dampens or switches off the transmission of stimuli between nerve and muscle cells, as the neurotransmitter acetylcholine responsible for this is released in smaller amounts or not at all. The result is sustainable muscle relaxation, the extent of which can be precisely controlled by the Botox dosage. Expression lines are effectively smoothed by injecting Botox, so that your face appears significantly rejuvenated and fresher afterwards.

Botox is primarily used for the treatment of forehead wrinkles, frown lines, and wrinkles in the eye area, but can also be used for wrinkle smoothing in the mouth and nose area, on the neck, or on the décolleté. Botox injections are particularly effective for superficially pronounced expression lines. For deeper wrinkles, better results can often be achieved by combining Botox and wrinkle injection with fillers.

What results can be achieved with a wrinkle injection with Botox or filler?

Through a wrinkle injection, you achieve a natural rejuvenation of your skin and facial contours in a minimally invasive way – i.e., without surgery. Even deep wrinkles are sustainably smoothed by the injection, superficial wrinkles are often no longer visible. Your appearance thus looks several years younger.

The treatment goals of a wrinkle injection can also include sustainable volume building and the modeling of the facial shape or individual facial areas – cheeks, lips, chin, nose, or jawline.

How permanent are the results of a wrinkle injection?

How long the result of a wrinkle injection lasts depends on the chosen treatment method and the substances used. Differences also exist with regard to the time at which the first results of the treatment become visible.

The results of a wrinkle treatment with hyaluronic acid last for an average of nine to twelve months. Of crucial importance for the durability are the depth of the injection, the hyaluronic acid preparation used, and the condition of the treated facial areas. Current hyaluronic acid products can now show an effectiveness of up to 14 months.

Wrinkle injections with poly-L-lactic acid have a durability of around two years. The result of a Liquid Lifting lasts for around 18 months. If Ellansé is used as a filler, it can last for several years.

With a fat transfer for wrinkle injection, only limited statements can be made about the durability of the treatment. It depends on the extent to which the injected fat cells integrate with the skin. If the complete autologous fat connects with the skin, the result of this wrinkle treatment can also be permanent.

The results of a wrinkle injection with Botox are visible for four to six months. Afterwards, a follow-up treatment is necessary. With regular injections, the intervals between appointments increase, as muscle tension in the treated areas is permanently reduced and unwanted mimic effects are also diminished.

All wrinkle treatments have in common that initial effects of the treatment are visible immediately after the treatment, but the final result only becomes clear over the course of a few days or – especially with poly-L-lactic acid – after a few weeks.

How much does a wrinkle injection cost in my Berlin practice?

Regarding the cost of a wrinkle injection, it depends on the chosen treatment method and the scope of the wrinkle treatment.

In my Berlin practice, wrinkle injections with hyaluronic acid, Botox, or poly-L-lactic acid are available from 250 Euros.

For a fat transfer for wrinkle injection, you must expect costs from 1,500 Euros. On the one hand, this is almost always a larger treatment, and on the other hand, the costs for liposuction and the preparation of the autologous fat also play a role.

How does a wrinkle injection in Berlin work?

In my Berlin practice, I perform wrinkle injections on an outpatient basis; a hospital stay is not required. Beforehand, an anesthetic cream is applied to the skin areas to be treated, so that the wrinkle injection proceeds without pain.

A minimally invasive wrinkle treatment is always performed with very fine cannulas, so you will hardly feel the injection except for a slight pulling sensation at the injection site. Patients who are very nervous and anxious can experience the treatment in a pleasant twilight sleep through light sedation.

The localization of the injection sites, the choice of active ingredient, and the dosage of the substances used are clarified beforehand during a detailed consultation and a physical examination.

A wrinkle injection with hyaluronic acid or poly-L-lactic acid takes between 20 minutes and one hour, depending on the scope of the treatment. A Botox injection is completed after just ten to 20 minutes.

For a Liquid Lifting, you must expect a treatment duration of 45 minutes to one hour. This form of wrinkle injection is usually performed in two to three sessions to achieve an optimal treatment result. Likewise, several treatments are recommended if more extensive corrections are to be made with another treatment method using fillers.

The treatment process of a fat transfer for wrinkle injection begins with liposuction, for which a local anesthetic is generally administered to safely eliminate pain. After an interruption, during which the fat cells are prepared, the autologous fat is injected into the facial areas to be treated. For the wrinkle injection, my patients can choose between the application of an anesthetic cream or a local anesthetic, just as with a Liquid Lifting. The liposuction takes about one hour, the wrinkle treatment is completed after 15 to 30 minutes.

What risks and unwanted side effects can occur with a wrinkle injection?

Wrinkle injections are low-risk treatments where you can expect hardly any undesirable side effects or complications. In my Berlin practice, I exclusively use natural-based fillers and very high-quality preparations for minimally invasive wrinkle treatments, so that intolerance reactions are almost impossible.

In the first hours and days after a wrinkle injection, redness, slight bruising, or swelling may occur at the injection sites, but these disappear after a short time and can be easily concealed with make-up.

To avoid undesirable side effects, it is very important that wrinkle injections are performed exclusively by a cosmetic surgeon who has extensive experience in the application of the various methods for wrinkle injections. This ensures that the result of the treatment optimally meets your wishes and expectations.

Is special aftercare necessary after a wrinkle injection?

A wrinkle injection does not require special aftercare. Minor discomfort directly after the procedure can be effectively relieved by cooling.

You are usually able to work and socialize immediately after the procedure. Sports and more strenuous physical activities are possible again after a few days. For a period of two weeks, you should avoid saunas, solariums, and cosmetic treatments.

Normally not. Contraindications against a wrinkle injection exist only in very rare cases. These can include, for example, acute skin diseases, infections, or some other medical conditions. During the individual preliminary consultation in my practice, we will, of course, clarify such questions.

A liquid lift is a special form of wrinkle injection with fillers, for which the same conditions apply as for other treatment variants. Here too, you will almost never have to reckon with exclusion criteria.

There is no general answer to this question. We will clarify which form of wrinkle injection is optimally suited for you in a detailed consultation. The individual findings of the patient determine the choice of therapy.

A wrinkle injection with hyaluronic acid, poly-L-lactic acid or autologous fat is particularly suitable for treating static facial wrinkles that arise from natural skin aging.
