What Are Fillers and Which Types Are Available?

Fillers are medical materials injected into the skin as a liquid or gel to create a smoothing and firming effect. They support and lift the skin from beneath, helping to restore lost volume.

Fillers such as hyaluronic acid and poly-L-lactic acid are based on substances that also occur within the human body and are generally well tolerated. However, the body gradually breaks them down through natural biological processes, so the anti-ageing effect eventually diminishes and treatment may need to be repeated.

In theory, minimally invasive wrinkle treatment can also be performed using non-biodegradable substances such as silicone, which may produce permanent effects. However, these materials no longer have an established role in anti-ageing treatment because they can cause foreign-body reactions and tissue changes. Biodegradable fillers generally carry a lower risk of these complications, although no injectable treatment is entirely free from risk.

Biodegradable Fillers for Wrinkle Injections

The principal biodegradable fillers used for wrinkle injections are hyaluronic acid, poly-L-lactic acid and your own fat. The most suitable option depends on individual factors such as your skin type, age and the depth of the wrinkles to be treated. Your personal preferences are, of course, also important. What Is Hyaluronic Acid?

Hyaluronic acid, sometimes simply called hyaluron, is a naturally occurring substance produced by the body. It is found in particularly high concentrations in the skin, muscles, joints and eyes. One of its principal roles is to regulate moisture within the body’s tissues at a cellular level. It also plays a part in modulating inflammation.

Hyaluronic acid can bind large quantities of water, helping to give the skin volume, elasticity and firmness. As the body produces less of it with age, wrinkles and volume loss become more apparent.

Hyaluronic acid injections work in two ways. First, the volume of the product fills and supports the wrinkle itself. Different Hyaluronic Acid Products for Wrinkle Injections

The selection of a particular hyaluronic acid product depends, for example, on whether the primary aim is to smooth wrinkles, reshape the tissues or restore volume.

The degree of cross-linking is another important consideration. Hyaluronic acid is available in non-cross-linked, lightly cross-linked and highly cross-linked forms. Highly cross-linked products are more elastic, are broken down more slowly by the body and are therefore used primarily for injections into deeper tissue layers. Lightly cross-linked hyaluronic acid is generally more suitable for anti-ageing treatments intended to smooth fine, crepey lines near the skin’s surface.

Wrinkle Injections with Poly-L-Lactic Acid

Like hyaluronic acid, poly-L-lactic acid (PLLA, Sculptra) is a biodegradable filler. It is manufactured from lactic acid, a substance also found in the human body, including in the blood and muscles. Poly-L-lactic acid injections stimulate the production of the body’s own collagen, helping to restore firmness and volume to the skin. This can soften deeper wrinkles and create a firmer appearance without surgery.

Non-Surgical Facelift in Berlin with a Liquid Facelift

Another injectable wrinkle treatment I offer in Berlin is the liquid facelift, which combines different injection techniques and fillers. Carefully selected injection points and depths are used to smooth wrinkles and reshape the face. In suitable patients, a liquid facelift can achieve improvements that once required surgery, although it cannot replicate every effect of a surgical facelift. Individual areas or the face as a whole can be treated.

Eight-Point Liquid Facelift

A specific variation of this anti-ageing approach is the eight-point treatment originating in Brazil (Profhilo), in which the entire face is treated through injections at eight key points selected to support a fresher, more youthful appearance. The injections are placed in the following areas:

• The zygomatic arch and cheek junction
• The chin and jawline
• The mouth and nose area, to soften nasolabial folds and lines between the corners of the mouth and chin

Fillers for a Liquid Facelift

A liquid facelift may use hyaluronic acid, Radiesse and, in some cases, poly-L-lactic acid (Sculptra). Radiesse is a biodegradable calcium-based filler used to treat deeper wrinkles and generally lasts longer than hyaluronic acid. During a liquid facelift, hyaluronic acid is primarily used for more superficial lines.

Ellansé is another innovative filler used in liquid facelifts. It is a synthetic, polycaprolactone-based substance with a particularly long duration and, like Radiesse, is well suited to treating deeper wrinkles. However, the tolerability of this biodegradable polyester filler has not yet been evaluated in long-term studies.

Wrinkle Injections with Your Own Fat in Berlin (Lipofilling)

As an alternative to hyaluronic acid treatment, I also offer wrinkle injections using your own fat in Berlin. The treatment takes place in three stages. First, fat is harvested from the thighs or another suitable area by liposuction and then carefully processed. The prepared fat is subsequently injected into the treatment area.

Fat transfer is particularly suitable for restoring volume across larger areas.

Wrinkle Injections with Botox

Botox wrinkle injections do not involve a filler. Instead, botulinum toxin type A, commonly known as Botox, reduces muscle activity to a degree determined by the dose. In its natural form, botulinum toxin is a potent neurotoxin. Products licensed for medical use are therefore manufactured under strictly controlled conditions.

To soften wrinkles, Botox is injected into the muscles whose contractions are to be reduced. It acts at the nerve endings, limiting communication between nerve and muscle cells by reducing the release of the neurotransmitter acetylcholine. This produces temporary muscle relaxation, which can be carefully controlled through the dose administered. Botox can effectively soften dynamic expression lines, helping the face look fresher and more relaxed.

Botox is primarily used to treat forehead lines, frown lines and wrinkles around the eyes. It may also be used for lines around the mouth and nose, on the neck or across the décolletage. Botox injections are particularly effective for superficial dynamic lines. For deeper wrinkles, combining Botox with dermal fillers can often produce a better result.

What Results Can Be Achieved with Botox or Filler Injections?

Wrinkle injections offer a minimally invasive, non-surgical way to create a naturally younger appearance by improving the skin and facial contours. Deep wrinkles can be softened with lasting results, whilst superficial lines may become barely visible. The face may appear several years younger, depending on your individual anatomy and the treatment performed.

Treatment goals may also include lasting volume restoration and the reshaping of the face or individual areas such as the cheeks, lips, chin, nose or jawline.

How Long Do the Results of Wrinkle Injections Last?

The duration of the results depends on the treatment method and substances used. The time at which the initial effects become visible also varies between treatments.

The results of hyaluronic acid wrinkle treatment last for an average of nine to twelve months. Longevity is influenced by the injection depth, the hyaluronic acid product used and the characteristics of the treated area. Some current hyaluronic acid products may provide results lasting up to 14 months.

Poly-L-lactic acid wrinkle treatment can last around two years. The results of a liquid facelift typically last approximately 18 months. If Ellansé is used, the effects may persist for several years.

It is more difficult to predict how long fat-transfer results will last. This depends on how much of the transferred fat establishes a blood supply and survives within the tissues. The surviving fat cells can provide a long-term or potentially permanent improvement.

The results of Botox wrinkle treatment are generally visible for four to six months, after which repeat treatment may be considered. With regular injections, some patients find that the intervals between appointments become longer as muscle activity in the treated areas remains reduced for longer periods.

With all wrinkle treatments, initial changes may be visible immediately, but the final result develops over several days or, particularly with poly-L-lactic acid, over several weeks.

How Much Do Wrinkle Injections Cost at My Berlin Practice?

The cost of wrinkle injections depends on the treatment method selected and the extent of the procedure.

What Happens During Wrinkle Injections in Berlin?

I perform wrinkle injections as an outpatient procedure at my Berlin practice, so no hospital stay is required. A topical anaesthetic cream is applied to the treatment areas beforehand to minimise discomfort during the injections.

Minimally invasive wrinkle treatments are performed using very fine needles or cannulas. Most patients feel little more than mild pulling or pressure at the injection site. If you are particularly anxious, the procedure can be performed under light twilight sedation where clinically appropriate.

I determine the injection sites, the most appropriate product and the dosage following a detailed consultation and physical examination.

Wrinkle injections with hyaluronic acid or poly-L-lactic acid take between 20 minutes and one hour, depending on the extent of treatment. Botox treatment usually takes around ten to 20 minutes.

A liquid facelift usually takes between 45 minutes and one hour. This form of wrinkle treatment is generally performed over two or three sessions to achieve the best possible result. Several sessions may also be recommended where fillers are used for more extensive treatment.

Treatment with autologous fat begins with liposuction, which is generally performed under local anaesthesia. The harvested fat cells are then processed before being injected into the areas of the face being treated. As with a liquid facelift, my patients can choose between a topical anaesthetic cream and local anaesthesia for the injections. Liposuction takes approximately one hour, whilst the facial injections usually take between 15 and 30 minutes.

What are the risks and potential side effects of wrinkle injections?

Wrinkle injections are generally low-risk treatments, although side effects and complications are possible. In my Berlin practice, I use high-quality, biocompatible fillers for minimally invasive wrinkle treatment, helping to minimise the risk of adverse reactions.

During the first few hours or days after wrinkle injections, temporary redness, mild bruising or swelling may occur at the injection sites. These effects usually subside quickly and can generally be concealed with make-up.

To reduce the risk of side effects, wrinkle injections should only be administered by an appropriately qualified doctor with extensive experience in the different injection techniques. Careful assessment, precise placement and appropriate product selection are essential to achieving a result that reflects your individual wishes as closely as possible.

Is any special aftercare required following wrinkle injections?

Wrinkle injections do not usually require any special aftercare. Applying a cool pack can help to ease mild discomfort immediately after the procedure.

You can generally return to work and social activities immediately after the procedure. Exercise and more strenuous physical activity can usually be resumed after a few days. You should avoid saunas, sunbeds and cosmetic treatments for two weeks.
